Cornerstone Rehab is seeking a full-time Speech Language Pathologist to deliver care to our patients and collaborate with a diverse team of clinical professionals. Qualifications

• Valid SLP License and Credentials.
• SNF and/or Acute care experience.

Responsibilities

• Provide physical therapy to adult and geriatric patients
• Prepare documentation, progress notes, and discharge summaries
• Establish a plan of care in accordance with the patients' needs and capabilities
• Travel to nearby centers as needed
