Job summary

Cornerstone Rehab is hiring a PRN Speech Language Pathologist to deliver care to our adult and geriatric patients and work with a diverse team of clinical professionals. A sign-on bonus of $750 is offered to join our team and contribute to excellent patient outcomes.

We provide support, resources, and professional development to help therapists succeed in a collaborative post-acute care environment. Travel to nearby centers is required as needed.

Qualifications

• Valid State SLP license.
• SNF and/or Acute care experience.

Responsibilities

• Provide physical therapy to adult and geriatric patients
• Prepare documentation, progress notes, and discharge summaries
• Establish a plan of care in accordance with the patients' needs and capabilities
• Travel to nearby centers as needed
