Job summary

Benchmark Therapy and Rehab is a large therapy practice providing services to a multitude of providers including Home Health, and we are seeking compassionate and driven Speech Language Pathologists/CCC-SLPs to join our therapy team in Ohio. We currently have both Fulltime (with Benefits) and consistent PRN available.

Position involves providing speech therapy services to patients in a home health setting by assessing patient needs and generating treatment programs with established goals.

Responsibilities

• Provides speech therapy services to patients and initiates referrals when appropriate.
• Reviews physician referrals, patient condition and medical records to determine if speech therapy is required.
• Evaluates patients to assess needs and conducts formal/informal assessments for treatment planning.
• Plans and implements individualized treatment programs with short and long‑term goals.
• Documents results of assessments, treatment, follow‑up and termination of services.
• Provides treatment in accordance with established standards of practice and departmental procedures.
• Demonstrates sound judgment in evaluation, planning, implementation and follow‑up.

Position Summary:

Position Responsibilities:

• Provides speech therapy services to patients and initiates referrals when appropriate.


• Reviews physician's referral (order), patient's condition, and medical records to determine that speech therapy treatment is required.


• Evaluates patients to assess patient needs, conducts formal/informal assessments necessary for treatment planning and implementation. Conducts specialized evaluations as indicated.


• Plans and implements individualized treatment programs by identifying a target list, including short and long term goals and methods to achieve identified goals.


• Documents results of patient's assessment, treatment, follow up and termination of services.


• Provides treatment in accordance with established standards of practice, department procedures, and productivity standards.


• Consistently demonstrates sound judgment in evaluation, planning implementation, and follow-up of resident therapy programs.
