Job summary

Asheboro Rehabilitation And Healthcare Center in Asheboro, NC is seeking a Speech Language Pathologist PRN to join our in-house rehab team and support resident care. The role focuses on comprehensive evaluations, treatment planning, and documenting progress per MD orders.

The position requires an MA/MS in communication disorders recognized by ASHA, current state licensure, and CFY completion, with a strong emphasis on collaboration and quality patient care.

Qualifications

• Graduate of an accredited university with an MA or MS in communication disorders recognized by ASHA.
• Holds current license and/or registration in the state.
• Successful completion of CFY.

Responsibilities

• Provide comprehensive speech therapy evaluations as per MD orders.
• Document findings on the standardized evaluation format neatly and accurately.
• Develop a comprehensive treatment plan with goals, frequency, duration, and modalities.
• Ensure MD orders are obtained for evaluations, treatments, recertifications and discharges.
• Provide treatment to patients using modalities and modify as indicated with precautions.
• Document progress, monthly treatment plans, and discharge information in approved formats.
• Attend facility meetings and participate in interdisciplinary team activities.
• Supervise junior staff or students in line with state/federal guidelines.

Qualifications

Job Responsibilities

• Provide comprehensive speech therapy evaluations as per MD orders.
• Document findings on the standardized evaluation format neatly, accurately and adhering to all guidelines.
• Provide a comprehensive treatment plan including long and short term goals, frequency, duration and treatment modalities, therapeutic interventions, clinical and technical guidelines.
• Ensure MD orders are obtained for evaluations, treatments, recertifications and discharges.
• Provide comprehensive treatment to patients utilizing modalities and modifying patient treatment as indicated and adhering to precautions.
• Document patient’s progress in approved format accurately and timely.
• Document monthly updated treatment plan in standardized format in timely manner.
• Document patient’s discharge and in-service as indicated.
• Document any caregiver or discharge in-services in approved format as indicated.
• Complete any and all other documentation required by Facility.
• Document care plans and information needed for the MDS as deemed necessary by the facility.
• Attend facility and rehab meetings as indicated by supervisor and/or Regional Manager.
• Participate in facility committee meetings as indicated.
• Act as a rehab delegate to the interdisciplinary team.
• Participates in scheduling of patients.
• Perform screens as indicated.
• Complete billing timely and bill patients ethically and accurately for all speech therapy services rendered.
• Cover other therapists’ patient caseload during absence as necessary.
• Supervise junior staff members or students in compliance with State ST Board and Federal regulations and requirements. Co-sign notes of CFY’s and students as indicated as per state and federal guidelines.
• Maintain professional relationship with co-workers, facility staff, patients and patient families.
• Provide in-service education regarding patient care or speech therapy service areas to rehab staff, facility staff and community as needed.
• Comply with all facility policies and procedures and safety guidelines.
• Comply with patient confidentiality and Federal Resident Rights and HIPAA legislation.
• Report to work on time, adhere to scheduled hours and project a professional image at all times.
• Adhere to productivity requirements, complete productivity reports and submit in a timely manner.
• Comply with all Facility Human Resource policies i.e. lunch, overtime, sick and time off notification.
• Maintain a positive work atmosphere by demonstrating and communicating in a professional manner so as to foster positive relations with customers, clients, co-workers and managers.
• Perform any other duties assigned by the department supervisor, lead therapist or Regional Manager.
• Adhere to any and all other Facility written and oral systems and operations.
