Job summary

Bolivia Rehabilitation and Healthcare Center in Bolivia, NC is seeking a Speech Therapist-PRN to join our rehab team. You will conduct evaluations, develop treatment plans, and deliver comprehensive therapy per physician orders, documenting progress and coordinating with the interdisciplinary team.

CFY completion or ASHA-certified practitioners with current state licensure are encouraged to apply. This role offers flexible scheduling and opportunities to work within a supportive care environment.

Qualifications

• Licensed speech-language pathologist or CFY completed under ASHA guidelines.
• Graduate degree in communication disorders recognized by ASHA/state boards.
• Current licensure/registration in the state of practice.

Responsibilities

• Perform comprehensive speech therapy evaluations per MD orders.
• Develop and document long- and short-term goals and treatment plans.
• Provide therapy using approved modalities and document progress timely.
• Ensure compliance with MD orders, HIPAA, and facility policies.
• Supervise CFYs or students as required and co-sign notes.

Qualifications

• Graduate of an accredited university with an MA, or M.S. in communication disorders that is recognized by ASHA and state board.
• Holds current license and/or registration in the state (as applicable).
• Successful completion of CFY.

Job Responsibilities

• Provide comprehensive speech therapy evaluations as per MD orders.
• Document findings on the standardized evaluation format neatly, accurately and adhering to all guidelines.
• Provide a comprehensive treatment plan including long and short term goals, frequency, duration and treatment modalities, therapeutic interventions, clinical and technical guidelines.
• Ensure MD orders are obtained for evaluations, treatments, recertifications and discharges.
• Provide comprehensive treatment to patients utilizing modalities and modifying patient treatment as indicated and adhering to precautions.
• Document patient’s progress in approved format accurately and timely.
• Document monthly updated treatment plan in standardized format in timely manner.
• Document patient’s discharge and in-service as indicated.
• Document any caregiver or discharge in-services in approved format as indicated.
• Complete any and all other documentation required by Facility.
• Document care plans and information needed for the MDS as deemed necessary by the facility.
• Attend facility and rehab meetings as indicated by supervisor and/or Regional Manager.
• Participate in facility committee meetings as indicated.
• Act as a rehab delegate to the interdisciplinary team.
• Participates in scheduling of patients.
• Perform screens as indicated.
• Complete billing timely and bill patients ethically and accurately for all speech therapy services rendered.
• Cover other therapists’ patient caseload during absence as necessary.
• Supervise junior staff members or students in compliance with State ST Board and Federal regulations and requirements. Co-sign notes of CFY’s and students as indicated as per state and federal guidelines.
• Maintain professional relationship with co-workers, facility staff, patients and patient families.
• Provide in-service education regarding patient care or speech therapy service areas to rehab staff, facility staff and community as needed.
• Comply with all facility policies and procedures and safety guidelines.
• Comply with patient confidentiality and Federal Resident Rights and HIPAA legislation.
• Report to work on time, adhere to scheduled hours and project a professional image at all times.
• Adhere to productivity requirements, complete productivity reports and submit in a timely manner.
• Comply with all Facility Human Resource policies i.e. lunch, overtime, sick and time off notification.
• Maintain a positive work atmosphere by demonstrating and communicating in a professional manner so as to foster positive relations with customers, clients, co-workers and managers.
• Perform any other duties assigned by the department supervisor, lead therapist or Regional Manager.
• Adhere to any and all other Facility written and oral systems and operations.
