Job summary

Alternate Solutions Health Network seeks a Speech Therapist to provide home-based skilled care to patients in a defined service area. You will educate patients and families, ensure safety, and collaborate with an interdisciplinary team to support recovery in the patient’s preferred environment.

The role emphasizes compassionate communication, adherence to clinical guidelines, and flexible scheduling to meet patient needs. A valid driver’s license and state licensure are required.

Qualifications

• Current license as Speech Therapist in the state of employment.
• Minimum one year of experience as a Speech Therapist in an acute care setting.
• Home care experience preferred.
• Valid driver's license and auto insurance in your name.

Responsibilities

• Plan of Care: Develop an individualized ST plan of care and adjust as needs change.
• Guide the patient in therapeutic and self-care activities to improve functioning.
• Observe, record, and report patient changes to attending physicians and agency staff.
• Improve swallowing, breathing, cognition, speech and communication through home programs.
• Minimize patient risk and reduce rehospitalizations through preventative education.
• Counsel patient and family on disease/injury and management across all therapy phases.
• Ensure clinical documentation follows agency protocol and Medicare guidelines.
• Follow agency policies, procedures, rules, and regulations; update schedules as needed.
• Attend in-service trainings and mandatory meetings.

SPEECH LANGUAGE PATHOLOGIST SCHEDULE: Full-Time, 5 days per week, Monday-Friday, 8:15 AM - 5:00 PM.

TERRITORY: Brewster, Greentown, Massillon, Navarre, Canton, Portage, Barberton.

HOW YOU'LL WORK

You’ll provide home-based skilled speech therapy care for patients as directed by the attending physician. You will educate patients and their family members and ensures the safety of the patient.

MAJOR AREAS OF RESPONSIBILITY

• Plan of Care: Develop an individualized ST plan of care and make adjustments as needs change.
• Guide the patient in his/her use of therapeutic, creative and self-care activities for the purpose of improving functioning.
• Observe, record, and report patient reactions, changes, and progress to attending physicians and any appropriate agency staff.
• Patient Outcomes: Use activities and home exercise programs to improve swallowing, breathing, cognition, speech and communication, and/or any other speech therapy related function.
• Minimize Patient Risk: Help decrease rehospitalizations by teaching the patient on preventative measures and making good decisions.
• Family Educator/Advocate: Counsel patient and family on the disease/injury and how to manage in all phases of speech therapy.
• Counsel and instruct patient and family on the disease/injury/disability and how to manage in all phases of speech therapy.
• Policies: Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
• Rules and Regulations: Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.
• Operations: Attend in-service training and mandatory agency meetings.

REQUIREMENTS

• Speech Therapist with a current license in the state of employment.

• Capable of all physical demands.
