Speech and Language Pathologist

The Center for Elders’ Independence is a PACE (Program of All-Inclusive Care for the elderly) organization that uses an interdisciplinary team approach for care planning and implementing purposeful high-quality, affordable, and integrated health care services to the elderly. Our participants meet PACE requirements as prescribed by CMS and are referred to as participants. Our PO includes adult day health centers and primary care clinics, promoting participant autonomy, quality of life and the ability for individuals to live in their communities.

PLEASE NOTE THIS IS A PER DIEM POSITION: 20 HRS PER WEEK SUPPORTING MULTIPLE CENTERS

The Position

The Speech Language Pathologist provides diagnostic and therapeutic intervention for participants with communication disorders, swallow disorders, and communication disorders in accordance with the Physician’s written authorization and in conjunction with the total plan of care.

The hourly rate for the Speech and Language Pathologist role at Center for Elders Independence is $41.40 to $62.10 per hour. The hourly rate is based on the market for the position as well as experience, skills, ability, and work history.

Duties and Responsibilities
  • Assists the Physician and the multidisciplinary team in evaluating agency participants’ level of communication skills and potential for rehabilitation or restoration, by applying diagnostic and prognostic procedures.
  • As a member of the multidisciplinary team, assists in establishing measurable functional goals and treatment plan to promote and maintain maximum level of speech and language function.
  • Administers appropriate treatment that may include auditory, visual, oral, speech, and visual-motor processes in accordance with the Physician’s authorization and the total plan of care.
  • Instructs multidisciplinary team and family on participant’s total speech and therapy program and phases of speech therapy with which they may work with the Participant.
  • Must reassess all participants who receive speech therapy at least every six (6) months.
  • Records quarterly progress notes on participants receiving speech therapy. The Speech Therapist must also document in the participant’s record all services scheduled in the care plan on the day services are provided.
  • Reviews and initials all evaluations and the discharge summary for participants receiving speech therapy.
  • Observes, records, and reports regularly to the multidisciplinary team on participants’ reaction to the speech therapy session and any changes in the participant’s condition.
  • Instructs, supervises, and evaluates staff in the implementation of the participant’s individual plan of care.
  • Provides in‑service training to other members of the health team.
  • Attends weekly multidisciplinary team conference as needed and actively participates in the development of the plan of care for participants receiving speech therapy.
Qualifications
  • Possession of a minimum of a Master’s degree in Speech Pathology.
  • Currently licensed as a Speech–Language Pathologist by the California Speech–Language Pathology and Audiology and Hearing Aid Dispensers Board.
  • Required, one (1) year of experience working with the frail or elderly population.
  • Current CPR and First Aid certification on file.
  • A valid California driver’s license, current vehicle insurance, and access to a reliable vehicle is required.
  • Ability to travel to multiple work sites and participants’ residences, required.
Physical Requirements
  • Frequent sitting, computer work, neck rotation.
  • Occasional standing, walking, kneeling, squatting, climbing, balancing, pushing/pulling, reaching, gripping, pinching.
  • Occasional lifting up to 10 lbs to/from floor, waist, shoulder, overhead; carrying at waist height.
  • Occasional lifting 11 to 50 lb at waist height.
  • May encounter walking on uneven ground and have exposure to biohazard.
