Job summary

Bridge Home Health and Hospice seeks a Speech-Language Pathologist to provide home-based services under our standards of care and service agreements. The role includes evaluating patients, developing plans of care, and delivering targeted speech therapy using appropriate techniques and equipment.

Ideal candidates will hold an SLP degree with state licensure, CPR certification, and a valid California driver’s license.

Qualifications

  • Master's or doctoral degree in Speech-Language Pathology (SLP) required; meets state licensure requirements.
  • Must be licensed as a Speech-Language Pathologist by the state or be in the process of licensure.
  • CPR certification required; valid California driver's license with insured vehicle.

Responsibilities

  • Conduct evaluations and assess home environment for speech-language function.
  • Identify communication deficits and plan care to improve patient function.
  • Administer speech therapy using specialized techniques/equipment.
  • Recommend communication devices/aids as indicated.
  • Coordinate with referring physician and care teams for comprehensive plans.
  • Prepare and submit notes per policy; maintain clinical competency.
  • Instruct patient and family in home treatment regimens; refer to other services as needed.
  • May supervise home health aide in therapy-related tasks.

Salary Range: $65.00 To $75.00 Hourly

Job Description Summary

The speech–language pathologist is responsible for implementation of standards of care for speech language pathology services and for adherence to all conditions in the Service Agreement.

Essential Job Functions/Responsibilities:
  • Conducts appropriate evaluations, assesses home environment (as appropriate), identifies equipment needs relative to speech/language function.
  • Identifies functional speech defects and establishes a plan of care to improve patient’s function.
  • Administers speech therapy program utilizing specialized therapeutic technique and/or equipment.
  • Recommends communication devices/aids as indicated.
  • Confers with referring physician and other organization personnel to ensure coordinated and comprehensive care contributes to the total plan of care.
  • As appropriate, participates in case conferences, in-services and other meetings required to ensure coordinated and comprehensive care.
  • Prepares and submits notes per organization policy.
  • Maintains clinical competency in speech language pathology practice and theory.
  • Identifies patient and family/caregiver needs for other services and refers as appropriate.
  • Instructs patient, family/caregiver and other Organization health care personnel in the patient’s treatment regime as indicated.
  • May supervise home health aide in procedures directly related to the speech–language therapy plan of care.
  • Performs other duties as required to facilitate the delivery of speech language pathology services.

Job Qualifications:
  • Possesses a master’s or doctoral degree in speech-language pathology (SLP), and who meets either of the following requirements:
  • Is licensed as a speech-language pathologist by the State in which the individual furnishes such service; or
  • If the State does not license speech-language pathologists:
  • Must successfully complete 350 clock hours of supervised clinical practicum (or is in the process of accumulating such supervised clinical experience);
  • Performed not less than 9 months of supervised full-time SLP services after obtaining a master’s or doctoral degree in SLP or a related field; and
  • Successfully completed an approved national examination in SLP
  • A minimum of one (1) year's clinical experience in Speech–Language Pathology.
  • Demonstrates good verbal and written communication, and organization skills.
  • Possesses and maintains current CPR certification.
  • Must possess a valid California Driver’s License with automobile that is in good working order and insured in accordance with the organization requirements.
Physical Requirements:
  • The ability to sit occasionally for 1-3 hours.
  • The ability to frequently stand for 3-6 hours.
  • The ability to occasionally drive/ operate foot control for 1-3 hours
  • The ability to walk occasionally on level surface for 1-3 hours
  • The ability to walk intermittently on unlevel surface for less than 1 hour
  • The ability to intermittently bend or climb for less than 1 hour.
  • The ability to occasionally twist, kneel, crouch and crawl for 1-3 hours
  • The ability to lift 50 pounds intermittently in less than 1 hour
  • The ability to carry up to 50 pounds intermittently in less than 1 hour.
  • The ability to push up to 50 pounds intermittently in less than 1 hour.
  • The ability to pull up to 50 pounds intermittently in less than 1 hour
  • Maintains current and valid Driver’s License and Automobile Insurance coverage at all times.
