Job summary

Desert Oasis Healthcare in Palm Springs, CA seeks a Speech Language Pathologist to develop and implement home health plans of care for patients with communication or swallowing difficulties.

You will conduct evaluations, coordinate with families and the care team, provide patient education, and ensure regulatory compliance while delivering compassionate care to geriatric patients.

Qualifications

  • Master's degree in Speech Language Pathology.
  • California SLP license required.
  • Doctorate in Speech Language Pathology preferred.
  • Experience with geriatric populations preferred.

Responsibilities

  • Evaluates home health patients and develops a plan of care based on home environment, patient needs, medical needs and equipment needs.
  • Performs initial and ongoing assessments including the OASIS when applicable.
  • Identifies functional speech and communication defects and collaborates with patient/family to meet goals.
  • Recommends communication devices/aids as indicated.
  • Participates in multi-disciplinary case conferences and QAPI when applicable.
  • Performs required direct/indirect patient care supervisory visits for Speech Pathologist Assistant per regulations.
  • Provides education to patient/caregiver regarding treatment and plan of care.
  • Reports changes in the patient's condition to the Physician and Therapy Manager.
  • Evaluates effectiveness of the plan of care and updates as needed.

Description

The Speech Language Pathologist(SLP) is responsible for developing and implementing a Plan of Care for Home Health patients with communication and/or swallowing difficulties.

Professional Duties
  • Evaluates home health patients and develops a plan of care based on home environment, patient identified needs, medical needs, and equipment needs.
  • Performs initial and ongoing assessments including the OASIS when applicable.
  • Identifies functional speech and communication defects and works with the patient/family to develop a plan of care that meets both the patient/family and agency goals.
  • Recommends communication devices/aids as indicated.
  • Participates in multi-disciplinary case conferences and QAPI when applicable
  • Performs required direct/indirect patient care supervisory visits for Speech Pathologist Assistant per regulations.
  • Provides health teaching to patient and/or caregiver regarding treatment and plan of care. Education will utilize terms and teaching methods that are customized to the learning needs of the patient and/or caregiver
  • Reports any change in the patient's condition to the Physician and the Therapy Manager.
  • Evaluates the effectiveness of the plan of care and updates as needed.

Qualifications
  • Master's degree in Speech Language Pathology. Doctorate degree in Speech Language Pathology, preferred. Current California Speech Language Pathologist License.
  • Valid California Drivers License and proof of auto insurance.
  • Experience working with geriatric patients, preferred.
  • Ability to multi task and prioritize to meet deadlines.
  • Good communication skills, detail oriented, critical thinking and problem solving skills.
  • Excellent customer service and telephone etiquette skills
  • BLS required within 60 days of hire
