Job summary

UVMMC Specialty Pharmacy - Clinical Services is seeking a Pharmacist to support the needs of Specialty Pharmacy patients. The Pharmacist will work in the Specialty Pharmacy and collaborate with multidisciplinary teams to resolve medication-related problems and dispense medications according to prescriptions while addressing financial barriers and counseling patients and health care professionals on proper use of medications.

The role requires a PharmD or B.S.

Qualifications

• PharmD or B.S. degree from ACPE-accredited college of pharmacy and Vermont pharmacist licensure.
• Minimum one year experience in retail/mail/call center/specialty/home infusion pharmacy within past 5 years.
• Minimum one year experience in health-system pharmacy preferred.

Responsibilities

• Triages and answers questions related to specialty pharmacy prescription fills and prior authorizations.
• Dispenses medication per prescription orders while screening for medication-related problems and barriers to access.
• Counsels patients and health care professionals on proper use of medications.
• Maintains sound business practices and clinical judgment to ensure department success and optimal patient outcomes.

Education

PharmD or B.S Pharmacy degree from ACPE accredited college of pharmacy
