Job summary

University of Vermont Medical Center in Vermont seeks a Specialty Operations Pharmacist to serve the needs of UVMMC Specialty Pharmacy patients. The pharmacist will work with multidisciplinary teams to resolve medication-related problems, dispense medications per orders, screen for issues, and counsel patients and health care professionals on proper use of medications.

• PharmD or B.S pharmacy degree from ACPE accredited college of pharmacy.
• Licensed pharmacist in Vermont.
• Minimum one year experience in a retail/mail/call center/specialty/home infusion pharmacy within past 5 years.
• Minimum one year experience in a health system pharmacy preferred.

Responsibilities

• Triage and answer questions related to specialty pharmacy prescription fills and prior authorizations.
• Dispense medications according to prescription orders and screen for medication-related problems.
• Counsel patients and health care professionals on proper use of medications.
• Collaborate with multidisciplinary teams to resolve medication-related problems and support department goals.
• Maintain sound business practices and clinical judgment to ensure patient outcomes.

Job Summary:

The Specialty Operations Pharmacist services the needs of the UVMMC Specialty Pharmacy patients. The pharmacist will work in the Specialty Pharmacy. The pharmacist will work with multidisciplinary teams to resolve medication related problems. Education:
