Qualifications

• PharmD or equivalent required; PGY-1 residency completed or 1+ year ambulatory care experience preferred.
• BCACP eligible or certified preferred.
• Active Pharmacist licensure in NY or eligibility required; state licensure within one year of hire.

Responsibilities

• Conduct comprehensive medication therapy reviews for specialty patients.
• Provide disease-specific counseling and education to improve adherence.
• Collaborate with physicians, nurses, and case managers for therapy initiation and continuity.
• Support prior authorization, appeals, and benefits verification to ensure access.
• Monitor outcomes, adverse events, and adherence metrics to optimize therapy.
• Document interventions and outcomes for quality improvement and audits.
• Ensure URAC/ACHC standards and payor requirements are met.
• Develop protocols and formulary strategies with the team.
• Assist in workflow optimization to improve patient satisfaction.
