Job summary

Clearway Health is looking for a Clinical Pharmacist to optimize outcomes for patients on chronic and specialty therapies. You will serve as a drug expert, educate pharmacists and staff, and coordinate acquisition and distribution when needed.

You will collaborate with physicians, nurses, and payors, perform medication reviews, and ensure adherence to standards and regulations. A PharmD or equivalent and residency experience are required.

Qualifications

• PharmD or bachelor’s degree from an accredited College of Pharmacy.
• Completion of PGY-1 Pharmacy Residency or 3+ years direct patient care is required.
• PGY-2 Ambulatory Care or Specialty Pharmacy Residency/Fellowship preferred.

Responsibilities

• Conduct comprehensive medication therapy reviews for patients on specialty meds.
• Provide disease-specific counseling and education to enhance patient understanding.
• Partner with prescribers, nurses, case managers, and payors to facilitate therapies.
• Support prior authorization, appeals, and benefits verification processes.
• Monitor clinical outcomes, adverse events, and adherence metrics.
• Document and report interventions to support quality improvement and compliance.
• Adhere to URAC/ACHC standards and manufacturer/payor requirements.
• Develop protocols and formulary strategies.
• Lead or support workflow improvements and program growth.
