Job summary

Saban Community Clinic is seeking a pharmacist to serve as the primary lead for the Specialty Pharmacy Program at our West Hollywood location. You will coordinate specialty med management from referral through follow-up, onboard patients, provide education and care planning, and support the pharmacy team.

Ideal candidates hold a PharmD and a current California license, with at least one year in specialty or ambulatory care.

Qualifications

• Doctor of Pharmacy degree required.
• Current California Pharmacist license in good standing.
• Minimum of one year of pharmacist experience in specialty/ambulatory care or integrated healthcare setting.

Responsibilities

• Serve as the primary pharmacist for the Specialty Pharmacy Program.
• Coordinate specialty medication management from referral through follow-up.
• Educate patients and provide care planning and reassessments.
• Monitor adherence, response, adverse effects, and medication-related problems.
