Job summary

Saban Community Clinic is seeking a highly qualified Pharmacist to lead chronic disease management and medication therapy in a community clinic setting. You will review patient histories in OCHIN Epic, optimize regimens, educate patients, and support specialty pharmacy processes.

The role emphasizes collaboration with a multidisciplinary team, documentation, and adherence to protocols to improve outcomes in diabetes, hypertension, and lipid disorders.

Qualifications

• A Pharm.D. degree is required and licensure in California is expected.
• Residency training or equivalent clinical experience in medication therapy management.
• Experience with diabetes, hypertension, and hyperlipidemia desirable.

Responsibilities

• Chronic disease management services as primary responsibility.
• Coordinate specialty medication management from referral through follow-up.
• Process medication refill requests per SCC protocols and ensure timely documentation.
• Participate in quality assurance and workflow improvement initiatives.
• Maintain current clinical knowledge through continuing education.
