Job summary

Qualifications

• Licensed California pharmacist required.
• Advanced Practice Pharmacist license strongly preferred.
• Board certification in relevant practice area preferred.

Responsibilities

• Provide comprehensive medication management for specialty/ambulatory care.
• Collaborate with providers and interdisciplinary teams to optimize therapy.
• Initiate, adjust, and discontinue therapies per guidelines; monitor labs.
• Educate patients on specialty medications and therapy outcomes.
• Develop and update protocols and participate in quality improvement.
