AndHealth LLC is seeking a Specialty Pharmacist to join a community health center in Illinois. The role emphasizes direct patient care, counseling, and evidence-based therapeutic recommendations for chronic conditions, along with ensuring regulatory and quality accreditation standards.

The successful candidate will have a PharmD or equivalent, an Illinois license in good standing, and a willingness to obtain licenses in other states.

Qualifikationen

• PharmD, BS in Pharmacy, or FPGEC (NABP).
• Unrestricted Illinois pharmacist license in good standing.
• Willing to obtain licenses in multiple states.
• 3+ years direct patient care experience preferred.
• BLS/CPR required (or willing to obtain within 1 month).
• Immunization delivery certification preferred.
• 2+ years of specialty pharmacy experience preferred.
• PGY1 residency or equivalent preferred.
• Excellent written and oral communication skills.
• Strong attention to detail and ability to multitask.
• Comfortable with technology and adapting to new systems.
• Travel may be required; reliable transportation needed.

Aufgaben

• Deliver direct care, therapeutic recommendations, and counseling for patients with chronic conditions.
• Review patient profiles for interactions, allergies, or duplications and adjust therapy as needed.
• Partner with Patient Engagement Specialists to secure patient access via payor formularies, prior authorizations, and appeals.
• Maintain documentation for URAC and ACHC accreditation and participate in Quality Management programs.
• Process, fill, verify, and dispense prescriptions while balancing workflows.
• Provide on-site or cross-site remote support to ensure continuity as needed.
• Other duties as assigned.
