Job summary

Qualifications

• Doctoral degree PharmD from an accredited college of Pharmacy required.
• 3+ years of direct patient care experience in a clinical role, preferably in a specialty pharmacy.
• Ambulatory care experience preferred.
• PHARMD - Licensed Pharmacist in Maryland or DC as applicable to practice site Upon Hire required and eligibility for reciprocity to surrounding states as required for practice Upon Hire required.

Responsibilities

• Collaborates with providers to ensure safe and effective use of specialty medications and improves communication among the patient's healthcare team.
• Conducts medication order verification, dosage regimen design, and drug therapy monitoring services.
• Creates and maintains specialty medication patient care plans and provides patient consultations.
• Documents services according to specialty pharmacy accreditation standards and payer specifications.
• Participates in collaboration with prescribers to advance patient care and monitor specialized therapies.
• Coordinates with clinical and administrative teams to improve efficiency and productivity.
