Full-time Pharmacist - Specialty Pharmacy Case Management

City/State: Norfolk, VA

Work Shift: Rotating; Mon-Fri with varying hours (8:00-16:30 / 8:30-17:00 / 9:30-18:00). One week of on-call duty every 10 weeks is required.

Overview

The Specialty Clinical Pharmacist is responsible for safe, effective, and prompt pharmacy services. They participate in all aspects of the medication-use system and provide individualized pharmaceutical care to patients within the assigned areas.

Responsibilities

• Demonstrates ability to work in pharmacy practice areas competently and independently.
• Manages patient drug therapy in accordance with applicable clinical practice standards.
• Assures appropriateness of all provider medication orders.
• Maintains current knowledge of medication use and disease states.
• Serves as the primary resource for drug information and education.
• Provides in-depth patient counseling when required.
• Participates in training of new employees as assigned.
• Assumes local responsibility and ownership for assigned tasks.
• Consistently follows policies and procedures.
• Seeks additional training opportunities and stays abreast of new information.
• Demonstrates superior customer service, effective communication, decision-making, and organization.
• Serves as a role model in professionalism, collaboration, and teamwork.
• Ensures adequate supplies for continuous patient care.
• Triages and escalates questions or issues beyond scope of practice.
• Uses appropriate systems for processing patient information.
• Ensures quality assurance per departmental and legal requirements.
• Operates pharmacy equipment appropriately.
• Precepts pharmacy students as assigned.
• In addition to the above responsibilities, other duties may be assigned.

Education

• Bachelor’s Level Degree in Pharmacy (Required)
• Doctorate Level Degree in Pharmacy (Preferred)

Certification / Licensure

• Virginia Pharmacist License (Required)
• Basic Life Support (BLS) (Required)
• Advanced Cardiac Life Support (ACLS) (Preferred)

Experience

• Previous Clinical Experience Preferred
• Drug Counseling Experience
• Knowledge of Specialty Medications
• PGY1/PGY2 Residency Trained Preferred
