Job summary

Veterans Health Administration (Department of Veterans Affairs) seeks a Specialty Case Management Registered Nurse to deliver coordinated patient care across inpatient and outpatient settings. Qualifications

• Current, full, active, and unrestricted RN license in a U.S. state or D.C.
• BSN or higher preferred for advanced roles.
• Experience in case management and outpatient/clinic settings.
• Strong English language proficiency for direct patient care.

Responsibilities

• Provide care coordination and discharge planning for inpatient and outpatient Veterans care.
• Collaborate with interdisciplinary teams to develop and implement care plans.
• Manage patient flow, triage, and appropriate follow-up within VA facilities.
• Support education and leadership within the CM nursing program.
