Veterans Health Administration, Department of Veterans Affairs, is seeking a Nurse Care Coordinator RN to provide and coordinate direct patient care, ensuring adherence to the nursing process and promoting education, self-management, and patient satisfaction across care settings.

The role emphasizes leadership in care coordination, collaboration with the inter-disciplinary team, and ensuring quality outcomes through evidence-based practice.

Qualifications

• English Language Proficiency
• Graduate of a school of professional nursing with ACEN/CCNE accreditation
• Active unencumbered nursing license in the United States
• Experience or education meeting VA Nurse qualification standards
• Understanding of safe patient handling and interdisciplinary collaboration

Responsibilities

• Provide and coordinate complex patient care planning and discharge/disposition planning for inpatient and outpatient settings
• Coordinate care with multiple providers across VA and community programs to ensure continuity of care
• Educate patients and families, promote self-management, and advocate for holistic care
• Lead and participate in interdisciplinary team activities to improve care outcomes
• Review and interpret laboratory and radiology results to inform care decisions
• Document outcomes and participate in policy/procedural improvements

Education

Graduate of a school of professional nursing with ACEN/CCNE accreditation
