Job summary

True Rx Health Strategists seeks a Specialty Care Coordinator to support patients and coordinate care across specialty medication programs. You will provide information to patients, pharmacies, and providers, and assist managers with obtaining medications and navigating prior authorizations.

You will collaborate with internal teams and external partners, document interactions in electronic records, and own issues through to resolution. Experience in healthcare or patient services is valued.

Qualifications

• High School graduate; some college preferred or 3+ years in customer service.
• Excellent written and verbal communication skills.
• Proficiency in Microsoft Office.
• Familiarity with HIPAA rules and patient confidentiality.
• Bilingual Spanish/English is a plus.

Responsibilities

• Provide information to patients, pharmacies, providers, and relevant parties about specialty medication programs, procedures, and benefit designs.
• Assist Specialty Care Managers in guiding patients through medication obtainment, specialty prior authorizations, and resources.
• Navigate various computer programs to retrieve information while assisting patients or team members.
• Collaborate with internal teams and external partners to resolve inquiries and ensure a seamless experience.
• Maintain detailed electronic records of interactions and actions taken.
• Take ownership of patient issues, following requests through to completion.
• Learn and retain benefit plan designs and Pharmacy Benefit Management knowledge.
• Adapt to changing procedures or policies with an open mindset.

True Rx Health Strategists is seeking dedicated individuals to join our Specialty Care Management team as Specialty Care Coordinators. In this pivotal role, you'll play a vital part in providing empathetic support to patients, collaborating with our team, and ensuring seamless coordination of patient care through our specialty medication programs.

Work in office: 40%; Work from home: 60%

What You'll Do:

• Patient-Centric Support:
• Accurately provide information to patients, pharmacies, providers, and relevant parties regarding specialty medication programs, procedures, and benefit plan designs.
• Assist Specialty Care Managers in delivering empathetic and compassionate care, guiding patients through medication obtainment, specialty prior authorizations, and available resources.
• Maneuver efficiently through various computer programs to retrieve information while caring for patients or assisting team members.
• Collaborate with internal teams and external partners to resolve patient or client inquiries, ensuring a seamless experience.
• Documentation and Ownership:
• Maintain detailed electronic records of each interaction, documenting actions taken to navigate the specialty medication process.
• Take ownership of patient issues, following requests through to completion and resolving problems in a timely fashion.
• Learn and retain benefit plan designs and Pharmacy Benefit Management (PBM) knowledge.
• Adapt easily to constant change, internalizing new procedures or policies with an open and receptive mindset.

What You Bring:

• High School graduate (some college preferred) or 3+ years of experience in customer service, patient care, or case management.
• Excellent written and verbal communication skills.
• Proficiency in Microsoft Office Products.
• Detail-oriented with strong organization and prioritization skills.
• Ability to multitask, strong analytical and problem-solving skills.
• Familiarity with current HIPAA rules and regulations to maintain patient confidentiality.
• Bilingual (Spanish/English) skills are a plus,
