Responsibilities

  • Personalised Care: Deliver specialised care to clients with specific conditions such as dementia, physical disabilities, or life-limiting illnesses.
  • Care Planning: Collaborate with healthcare professionals to develop and implement tailored care plans that meet individual client needs.
  • Health Monitoring: Observe and report changes in clients' health, ensuring timely intervention and adjustment of care plans as necessary.
  • Medication Management: Assist with administering medications in accordance with prescribed guidelines and training.
  • Emotional Support: Provide compassionate support to clients and their families, addressing emotional and psychological needs.
  • Advocacy: Act as an advocate for clients, ensuring their preferences and rights are respected and upheld.
