Responsibilities

  • Connect students and families with relevant community resources, agencies, and postsecondary institutions (DDS, BRS, etc).
  • Consult with staff prior to PPT meetings to ensure IEPs with transition goals meet students’ needs.
  • Collaborate with school staff, families, and external agencies for coordinated transition planning.
  • Assist in development and implementation of individualized transition plans.
  • Meet with regional transition coordinators and engage in RESC trainings/workshops.
  • Facilitate job shadowing experiences, internships, part-time job searches, career speakers, and transition-focused community outings.
  • Develop and maintain relationships with community businesses and agencies for job opportunities.
  • Facilitate referrals to local/state agencies, transition programs as needed.
  • Develop and implement department-wide programming aligned with CT CORE Transition Skills.
  • Work with staff to develop recommendations based on evaluations for postsecondary planning.
  • Conduct and report evaluations to develop CT CORE alignment.
  • Facilitate person-centered planning for students and families.
  • Lead staff in implementing transition goals and objectives.
  • Provide direct instruction to students to develop transition skills.
  • Create and manage on-campus work site opportunities.
  • Maintain records of transition activities and contribute to program evaluation.
  • Case manage students.
  • Develop and lead transition-focused trainings for the department.
  • Complete transition paperwork in relation to state policies/practices.
  • Other duties as assigned by administration

Special Education Supervisor, Building administrators

PERFORMANCE RESPONSIBILITIES: (As Appropriate)

  • Connect students and families with relevant community resources, agencies, and postsecondary institutions (DDS, BRS, etc)
  • Consult with staff prior to Planning and Placement Team (PPT) meetings to ensure the development of IEPs with transition goals, accommodations and modifications are designed to meet each student’s unique needs
  • Collaborate with school staff, families, and external agencies to ensure a coordinated approach to transition planning (serve as direct liaison, organize parent workshops and trainings)
  • Assist in the development and implementation of an individualized transition plan for Special Education students
  • Meet with regional transition coordinators and engage in RESC provided trainings/workshops.
  • Facilitate job shadowing experiences, internships, part-time job searches, career speakers, and transition-focused community outings
  • Develop and maintain working relationships with community businesses, agencies, and organizations to develop job site opportunities
  • Facilitate appropriate referrals to local and state agencies, transition programs on a case-by-case basis
  • Develop and implement department-wide programming and curriculum aligned with the Connecticut CORE Transition Skills (including, but not limited to, daily living skills, prevocational and vocational skills, postsecondary planning and career exploration)
  • Work closely with staff (teachers, administrators, and school counselors) to assist in developing recommendations, based on evaluation findings, for postsecondary planning
  • Conduct and report on evaluations to develop CT CORE Transition Skills and Standards-Based alignment
  • Facilitate person-centered planning for students and families
  • Lead staff in the implementation of transition goals and objectives
  • Provide direct instruction and support to students to develop essential transition skills such as self-advocacy, and job readiness (transition courses, 1:1 service time, self-contained push-in)
  • Create and manage on-campus work site opportunities
  • Maintain accurate records of transition activities and contribute to program evaluation
  • Case manage students
  • Develop and lead transition-focused trainings for the department
  • Complete transition paperwork in relation to state agency and state-mandated transition policies/practices
  • Other duties as assigned by administration
