Job summary

WhiteCap Search is seeking a Spanish-speaking NP or PA to support Value-Based Care and ACO initiatives in New Jersey. The role blends telemedicine with office-based care, partnering with the quality team to improve Star Ratings and patient outcomes.

The position requires an active NJ NP/PA license, primary care experience, and Spanish fluency, with a schedule Monday–Friday, 8:00 AM–5:00 PM. You will work across multiple primary care practices with strong administrative support.

Qualifications

• Active NJ NP or PA license (or equivalent).
• Primary care experience is preferred.
• Spanish fluency is required.

Responsibilities

• Conduct telemedicine visits for established patients identified through ACO and value-based care gap reports.
• Collaborate with Care Quality team to address preventive care, chronic disease management, and quality measure gaps.
• Act as an extension of patients’ PCPs for those unable or unlikely to attend routine visits.
• Spend 2 full days per week performing telemedicine visits with support staff at an office location.
• Rotate among primary care practices for the remaining 3 days per week as a float provider.
• See approximately 18–22 patients per day.
