Job summary

iCare Health Network in Hartford, CT is seeking a full-time Social Worker, preferably Spanish speaking, to coordinate and supervise the delivery of social work services for residents.

Responsibilities include interviewing residents, assessing cognitive and psychosocial needs, planning discharges, responding to emergencies, and collaborating with the interdisciplinary team to connect families with community resources and support.

Bachelor’s degree in Social Work or a related field is required.

Qualifications

• Bachelor’s degree in Social Work or related human services field, required.
• A minimum of one year’s experience in long-term care providing direct social services; Spanish speaking preferred.

Responsibilities

• Conduct face-to-face resident interviews and assessments of cognitive, mood, behavioral status and other psychosocial factors.
• Participate in planning for resident discharges.
• Respond to clinical emergencies, regulatory visits, surveys and issues outside normal work hours as needed.
• Direct and supervise the delivery of medically related social services to all residents and families.
• Assist in clinical evaluation and screening of potential admissions.
• Collaborate with the interdisciplinary treatment team to access community-based services.
• Serve as liaison with contracted mental health practitioners and consultants.
• Develop and implement new behavioral and psychosocial service programs and present to physicians, families, and residents.
