Qualifications

• AA/AS/BA in human services, psychology, or related field.
• At least one year of case management or psychiatric treatment experience.

Responsibilities

• Conduct client intake and assess needs and life situations.
• Develop individualized treatment plans with clients and families.
• Coordinate and manage services to implement care plans.
• Provide counseling, crisis intervention, and skill-building services.
• Maintain regular client contact and documentation per regulations.
• Meet monthly productivity requirement (100 hours).
• Transport clients as needed (personal vehicle required).
• Engage in outreach and referrals for non-enrolled individuals.
