Job summary

Clarvida is seeking a Recovery Coordinator at our Tehachapi location to provide outreach, information-referral, case management and a full array of mental health services to clients, including counseling, crisis intervention and treatment planning. The role may involve leading other staff and requires strong organizational and communication skills.

Qualifications

• Valid California driver’s license and auto insurance; CPR certification required.
• Minimum one year of case management or psychiatric treatment experience.
• Knowledge of rehabilitation model.
• Excellent prioritization and organization skills.
• Strong interpersonal and written and verbal communication skills.
• Intermediate level PC skills.
• Related experience: coaching, scouts, teacher’s aide, or CPS aid.

Responsibilities

• Conduct intake and comprehensive evaluation; prepare a detailed plan of care with client participation.
• Coordinate services by linking to community resources and monitoring plan effectiveness.
• Provide professional support including home visits, guidance and intervention as needed.
• Transport clients using personal vehicle; local travel required.
• Meet monthly productivity requirement of 100 hours.
• Maintain daily activity records and billing forms per standards and Medi‑Cal rules.
• Perform outreach and information/referral services for clients not requiring case management.
• Maintain HIPAA compliance and professional boundaries with staff and clients.
• Provide temporary intervention to ex-clients as needed.
• Develop relationships with agencies and service providers.
• Participate in meetings and training; perform other duties as assigned.
