NewYork-Presbyterian Hospital is seeking a Health Priorities Specialist – Patient Navigator (Evenings) who speaks Spanish to guide patients through the continuum of care. You will coordinate the patient experience from ER to discharge, educate patients about health issues, and connect them with community resources to support continued care.

Qualifications

• Bachelor's degree required.
• Fluent in Spanish required.
• Minimum of 2 years' experience in primary or community health.
• Knowledge of hospital and community resources.

Responsabilités

• Serve as a patient ally throughout the continuum of care from ER to discharge.
• Coordinate the full breadth of the patient experience for each patient.
• Direct patients to appropriate specialists or services and connect with community resources.
