Job summary

Doral Health & Wellness seeks a Health Home Enrollment Specialist to connect eligible individuals with the Health Home program across New York City. You will guide patients through enrollment and coordinate with care managers to ensure access to medical, behavioral health, and social services.

The role requires on-site work five days a week and travel across the five boroughs. Strong communication, organizational skills, and experience with health home enrollment are valued.

Qualifications

• High school diploma or equivalent required; Associate's or Bachelor's degree preferred.
• Experience in health home, healthcare enrollment, intake, or case management support is a plus.
• Knowledge of Medicaid guidelines, managed care, or community health programs is beneficial.
• Strong communication, interpersonal, and organizational skills are essential.
• Proficiency in Microsoft Office and EHR or database systems (ePACES, MAPP, eCW, PSYCKES, FCM).
• Ability to travel across the five boroughs for home and community visits via public transportation.

Responsibilities

• Conduct patient outreach and intake for health home programs and ensure timely engagement.
• Educate patients and families about health home services, benefits, and eligibility.
• Document outreach, eligibility verification, and enrollment activities in the care management platform.
• Complete enrollment applications and ensure documentation is accurate and compliant with regulations.
• Verify insurance coverage and Medicaid eligibility; obtain required authorizations.
• Collaborate with case managers and providers to activate services timely.
• Maintain organized records and update EHR systems.
