Job summary

VNS Health is seeking a Care Coordination Support role in New York to assist Care Coordinators and staff in resolving member-related issues. You will communicate with members and families, educate on plan features, and coordinate intake for clinical and non-clinical inquiries.

You will help monitor member satisfaction, document issues, and escalate clinical concerns as needed. The role emphasizes confidentiality and regulatory compliance within a large nonprofit health system.

Qualifications

• Education: High School Diploma or equivalent required.
• Associate's Degree in a healthcare related field preferred.
• Work Experience: Minimum of one year of customer service experience required.
• Language Interpreter Only: Ability to read, write, and speak the English language and their native or target language.

Responsibilities

• Works with members, providers, and staff to identify and resolve member service and retention related issues.
• Coordinates telephonic intake and responses to both clinical and non-clinical customer service issues.
• Assists in monitoring member satisfaction through phone calls and documents issues.
• Maintains expertise in MLTC benefits and guides members in accessing their benefits.
• Places orders or referrals for services and follows up to ensure services are scheduled.
• Serves as a resource for MLTC Care Coordinators and other staff in resolution of member-related issues.
