RiseWell Community Services is looking for a Full-Time Team Leader to join the SOS Team in Brooklyn, NY
In this position, you will be responsible for the implementation and maintenance of the program model. Oversee daily operations of the critical support team and program. This includes but is not limited to assisting the team with: community outreach, specifically on the streets and subways, coordinating participants needs before and after moves (daily living skill enhancement, accompany to appointments, advocacy, etc.). Responsible for the development and maintenance of collaborative relationships with community partners including the Department of Homeless Services, MTA, local law enforcement, community hospitals, existing outreach teams and housing providers.
Master’s degree or higher in Social Work, Mental Health Counseling, Public Administration, Nursing, Public Health, Public Policy or related field. Minimum of 3 to 5 years post-masters work experience in the behavioral health and criminal justice field. Experience with target population preferred. Previous management experience preferred. Knowledge of NYC subway system. Fingerprinting, criminal record check, approval from NYS Office of Mental Health.
