Responsibilities

• Coordinates care of solid organ transplant patients and donors.
• Monitors postoperative function and complications.
• Provides discharge instructions and coordinates post-transplant follow-up.
• Educates patients and families across the continuum of care.
• Collaborates with interdisciplinary team to resolve care issues.
