Job summary

Spinal Cord Living-Assistance Development, Inc. (SCLAD) seeks a professional Social Worker/Case Manager to join its team in Hialeah, FL. The CM will engage Program Participants, assess needs, develop ISS, and connect them with community resources to support independent living.

The role emphasizes advocacy, collaboration with families and providers, and maintaining accurate documentation of progress. Fluency in English and Spanish is preferred.

Qualifications

• Bachelor's or Master's degree in Social Work or related field.
• Experience in case management, disability services, or rehab is a plus.
• Knowledge of Florida Medicaid/Medicare and social service systems.

• Bilingual English/Spanish preferred for the Hialeah, FL community.

About the Role:

SCLAD (Spinal Cord Living-Assistance Development, Inc.) is a community-based organization that serves persons with physical disabilities to promote independent living. SCLAD is looking for a professional Social Worker/Case Manager (CM) to join its team in Hialeah. In this challenging role, the CM will make a significant impact in the lives of Program Participants (PP) by connecting them with the resources and support they need to thrive; and to the greater community in which they live. This is a very challenging and rewarding position.

Responsibilities:

• In-depth intake interviews, basic social assessments to ascertain specific needs

• Development of an Individual Service Strategy (ISS) based on intake data and assessment

• Implement ISS, in conjunction with PP, by identifying and connecting PP with resources in the community that respond to the ISS.

• Develop, with PP, a reliable support network that may include family, friends, healthcare providers, therapists and other professionals

• Monitor PPs’ continuum of care; quality of support and services that form part of support network

• Document and report on PP’s progress; updating ISS as needed.

• Advocate on behalf of PM to promote access to services and successful rendition of same

Requirements:

Bachelor's (or Master's) degree in Social Work (BSW/MSW) or related field

Prior experience in case management, disability services, or rehabilitation settings a plus

Strong knowledge of community resources, Medicaid/Medicare, and social service systems in Florida

Excellent communication, organizational, and problem-solving skills

Bilingual (English/Spanish) preferred given the Hialeah, FL community

Empathetic, patient, and client-centered approach to care
