Job summary

St. Croix Hospice seeks a compassionate Social Worker to join the patient care team. You will meet with patients and families, collaborate with the clinical staff, and help access community resources as outlined in the Plan of Care.

The role emphasizes psychosocial assessments, counseling, bereavement support, and coordination of care while maintaining HIPAA compliance and documentation. A BS in social work and state licensure are required, with multiple licensure levels offered for career

Qualifications

• Bachelor’s degree in social work from an institution accredited by the CSWE.
• Current Social Work license in the state(s) of practice.
• Five licensure levels: CSW, CAPSW, CISW, LCSW, SWTC.
• Ability to pass background, consumer report, and drug test.
• Conduct psychosocial assessments, identify safety or support needs, and provide resources, referrals, and care planning support for patients and families.
• Provide counseling, crisis intervention, and bereavement support, including follow up for at risk individuals and coordination of bereavement activities.
• Collaborate with the interdisciplinary team, maintain accurate documentation, support cultural and end of life needs, liaise with community agencies, and ensure HIPAA compliance.
• Must have reliable transportation, valid driver’s license, and proof of auto insurance
