NHC HealthCare Milan is seeking a compassionate and dedicated Social Worker to join our team in a temporary position through January 1, 2027.
The qualified candidate should hold a Bachelor’s or Master’s degree in Social Work from an accredited program and have knowledge of the case management process and the needs of geriatric patients, including healthcare funding resources.
This position is an excellent opportunity for someone who enjoys advocating for patients and families while working collaboratively with an interdisciplinary healthcare team.
Responsibilities include, but are not limited to:
Work with individual patients and families as they adjust to center life and cope with illness, disability, and end-of-life concerns
Serve as an advocate for patients and families in expressing, defining, and resolving concerns or grievances
Coordinate discharge planning functions and assist with safe transitions of care
Participate in developing patient care plans in cooperation with other healthcare disciplines
Communicate the current status and needs of Social Services to the Director of Social Services
Assist patients and families with understanding available healthcare and funding resources
Maintain confidentiality of patient and professional information
Work cooperatively with nursing, therapy, administration, and other members of the interdisciplinary team
Bachelor’s or Master’s degree in Social Work from an accredited college or university
Knowledge of the medical case management process and the needs of geriatric patients
Working knowledge of healthcare funding resources such as Medicare Parts A, B, C, and D, Medicaid, Managed Care, and private insurance
Strong organizational, communication, and interpersonal skills
Ability to work effectively and collaboratively with patients, families, and healthcare professionals
Insurance authorization experience is preferred
