Qualifications

• Bachelor’s or Master’s degree in Social Work from an accredited program.
• Knowledge of the medical case management process and the needs of geriatric patients.
• Understanding of healthcare funding resources (Medicare Parts A, B, C, and D; Medicaid; Managed Care; private insurance).
• Strong organizational, communication, and interpersonal skills.
• Insurance authorization experience is preferred.

Responsibilities

• Work with individual patients and families as they adjust to center life and cope with illness, disability, and end-of-life concerns.
• Serve as an advocate for patients and families in expressing, defining, and resolving concerns or grievances.
• Coordinate discharge planning functions and assist with safe transitions of care.
• Participate in developing patient care plans in cooperation with other healthcare disciplines.
• Communicate the current status and needs of Social Services to the Director of Social Services.
• Assist patients and families with understanding available healthcare and funding resources.
• Maintain confidentiality of patient and professional information.
• Work cooperatively with nursing, therapy, administration, and other members of the interdisciplinary team.
