Overview
Enloe Health Rehabilitation Center is a 40-bed acute rehabilitation unit dedicated to helping patients who are recovering from various injuries or illnesses. In partnership with Lifepoint Rehabilitation, this facility provides 24 hours of nursing care and intensive inpatient therapy to deliver outstanding experiences and quality outcomes for all of the patients we serve.
Responsibilities
- Interviews and assesses patients and/or patients' families, caregivers, and/or legal representatives.
- Determines, prioritizes, provides and/or arranges for needed internal and external services or interventions.
- Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with the patient\'s treatment plan.
- Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.
- Performs Medicaid screenings as indicated for the NHP.
- Assists patients with Safety Net applications when needed.
- Monitors and evaluates the quality and appropriateness of social services on inpatient behavioral health units in accordance with accepted standards of practice and established policies and procedures.
- Conducts a bio-psycho-social assessment of the patient and participates in the treatment planning process, with ongoing reassessment and evaluation.
- Formulates individualized, measurable goals for the patient.
- Participates in discharge planning, arranging for follow-up care, continuity of care, and coordination with outside agencies.
- Uses various modalities such as group therapy and individual therapy to treat the patient and assist families.
- Participates in utilization management activities as needed to ensure proper payment for services.
- Completes required documentation within required time frames.
What We're Looking For
- Education: Master\'s degree required
- Licenses: State of Michigan Licensed Master\'s Social Worker (LMSW)
- Certifications: Basic Life Support Health Care Provider (BLS-HCP); Crisis Intervention Training (within 90 days of hire/transfer)
- 2 years related experience
- Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision
- Must be able to work in a stressful environment and take appropriate action
Pay
Pay wage: $32.64-$59.16
