Lifepoint Rehabilitation, part of Lifepoint Health, is seeking a Social Worker for the Acute Rehabilitation Unit (ARU) at Baylor St. Luke's Medical Center – McNair Campus in Houston. The role focuses on supporting patients recovering from serious conditions with comprehensive social work interventions.

Join a 30-bed inpatient rehab unit that emphasizes patient-centered care, discharge planning, and collaboration with the healthcare team to promote recovery and independence.

Qualifications

• Education: Bachelors required; Masters preferred.
• Licenses: Board Certified Social Worker, or related field (i.e. counseling)
• Certifications: Handle With Care within 30 days of hire.
• Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.

Responsibilities

• Assesses, and plans interventions to help patients cope with social, emotional, economic, and environmental problems.
• Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives.
• Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.
• Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.
• Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.
• Supervises and/or trains new staff, students, and interns.
• Provides individual, family and group therapy.
