Job summary

Your Health, a leading physician group serving South Carolina and Georgia, seeks a compassionate Social Worker to support patients across Sumter and surrounding areas. This role involves home and facility visits, advance care planning, cognitive assessments, and resource coordination within a dedicated care team.

You must have reliable transportation as travel is required daily. Full-time, 8-hour shifts, Monday–Friday, 8am–5pm, with opportunities for growth and competitive benefits.

Qualifications

• Experience as a Social Worker in healthcare settings.
• Excellent customer service and compassionate bedside manner.
• Ability to travel daily to homes and facilities in service area.
• Strong communication with patients, families, and care teams.

Responsibilities

• Initiating Advance Care Planning for patients throughout their assigned Care Team (including facilities).
• Completing Daily Comprehensive Assessments on patients.
• Completing Daily Care Plans on patients.
• Documenting Chronic Care Management time daily.
• Meeting Daily Productivity Goals
• Coordinating discharges at contracted facilities
• Coordinating Placement.
• Continuation of coverage (i.e. coordinating acute visits for falls, med changes, etc.)
• Build and maintain professional relationships with referral sources.
• Maintain strong communication and professionalism between referral sources, patients/families, and Care Teams.

Job description

We are seeking a qualified Social Worker to assist our patients in our Sumter area. This candidate should be a compassionate Social Worker with excellent customer service skills to assist our patients' multiple facilities and homes in the area.

This role will perform visits in homes and facilities (ALF and ILF's) in their designated service area. You must have reliable transportation as travel is required daily. This is a full time, salary-based working 8-hr shifts Monday-Friday (8am-5pm)
