Job summary

The Veterans Health Care System of the Ozarks in Fayetteville, AR seeks a Social Worker (Program Coordinator) to evaluate and coordinate admissions for SUD and mental health services, ensuring timely, appropriate care for Veterans.

The role involves conducting psychosocial assessments, guiding referrals, and collaborating with Veterans Justice Outreach, clinicians, and family members to support recovery and transition through outpatient to residential programs.

Qualifications

• US Citizen is required to apply.
• Must be proficient in written and spoken English.
• Subject to background/security investigation.
• Must complete online onboarding and provide required documents.
• REAL ID compliant ID required after May 7, 2025.

Responsibilities

• Evaluate and coordinate referrals to the appropriate level of care (outpatient, intensive outpatient, sober living, residential) for Veterans.
• Coordinate day-to-day admissions/intake for multiple programs and ensure compliance with prescribers, nurses, and addictions therapists.
• Conduct psychosocial assessments, Suicide Screening/Comprehensive Suicide Risk Assessment, and discharge planning as part of comprehensive care.
• Collaborate with Veteran Justice Outreach, family members, and other providers to facilitate timely services and resources.
