Job summary

CentraCare Health is seeking an inpatient care management regional float social worker to support multiple CentraCare hospitals. You will manage a caseload, collaborate with an interdisciplinary team, and plan care to meet transitional needs for patients and families.

The role offers a full-time daytime schedule onsite, with competitive hourly wages and sign-on incentives. Candidates should hold a Minnesota social work license and ACMA CM-SW certification within five years.

Qualifications

• Current Licensed Social Worker (LSW) in Minnesota required.
• Bachelor's or Master's degree in Social Work required.
• 2+ years of clinical social work experience.
• Valid driver's license with good driving record.
• ACMA Case Manager-SW Certification within 5 years.

Responsibilities

• Maintain a designated patient caseload and plan care to meet transitional needs.
• Collaborate with an interdisciplinary team of professionals.
• Support patients and families during challenging times and coordinate resources.
• Assist with discharge planning and community resource referrals.

Join the inpatient care management team as a regional float social worker.This unique role will have the ability to support multiple CentraCare hospitals as a float social worker.

Our Socials Workers serve patients and families in our community and beyond by helping them during a challenging time in their lives. Our Social Workers enjoy the variety of cases they are presented with as there is always something new and different. You'll collaborate with other Social Workers to gain knowledge and experience and work with an interdisciplinary teamof nurses, therapists, physicians, and dieticians. You will have a designated patient caseload and will plan care to effectively meet the patient/family transitional needs including monitoring the length of stay and promoting community resources.

Schedule

• Full-time | 80 hours every two weeks | Onsite


• Day shift | Monday through Friday | 7:30am-4:00pm

Pay & Benefits
