Job summary

VOA-Greater New York is seeking a Social Worker to support formerly homeless clients in Permanent Supportive Housing across The Bronx and Brooklyn. You will coordinate with case management, recreational, and operations staff to serve assigned sites and maintain thorough documentation.

Minimum qualifications include a Master's in Social Work, LMSW licensure within 1 year of hire (exam costs reimbursed), and at least 2 years' experience with adults with serious mental illness and/or homelessness.

Qualifications

• Master's Degree in Social Work with LMSW licensure within 1 year of hire; reimbursement for LMSW exam costs.
• Minimum 2 years experience with adults with serious mental illness and/or substance use disorder and/or homeless populations.
• Familiarity with behavioral health treatment approaches including harm-reduction models.
• Strong teamwork, communication, and client-centered practice.

Responsibilities

• Conduct initial and follow-up assessments; manage caseload for routine check-ins, clinical case management, counseling, and behavioral health navigation.
• Develop and facilitate therapeutic and psychoeducational group programming.
• Maintain thorough service documentation and coordinate referrals, warm handoffs, and crisis support as needed.
