Job summary

Lifepoint Health is seeking a Social Worker for PRN coverage at the Towanda campus. The role focuses on assessing social, emotional, and environmental needs, planning interventions, and coordinating services to support patients and families.

Responsibilities include interviews, discharge planning, patient referrals, and providing therapy. A bachelor’s degree is required; a master’s is preferred, with state licensure within six months of hire.

Qualifications

• Bachelor's degree required; master's preferred.
• Licensure as Social Worker in state of Kentucky or eligible for licensure within 6 months of hire.
• Handle with Care certification within 30 days of hire.

Responsibilities

• Interviews and assesses patients and families to determine needs.
• Develops and implements plans for internal and external services/interventions.
• Participates in case reviews and coordinates with healthcare team for care plan adherence.
• Assist with discharge planning and connect patients to resources and transportation.
• Trains and supervises staff, students, and interns.
• Provides individual, family and group therapy.
