Job summary

Tennessee Quality Care Hospice in Martin, TN seeks a licensed PRN Social Worker to join our growing team. The role involves counseling, resources, and coordination with the clinical team to ensure holistic home-based care for patients and families.

MSW and Tennessee state licensure are required. Schedule is As Needed with shifts typically Mon-Fri 8:00am-4:30pm. Qualifications include 1+ year in health care or hospice and valid driver's license.

Qualifications

• MSW degree required and TN state licensure (LMSW or LCSW).
• Minimum 1 year of health care or hospice experience.
• Valid driver's license and proof of insurance.

Responsibilities

• Perform psychosocial assessments for patients and families in the hospice program and integrate needs into the plan of care.
• Provide counseling to patients, families, and staff to address stress and coping difficulties.
• Document patient services accurately and timely to reflect the plan of care.
• Assist with applications for SSI, Hospice, Medicaid, and Medicaid Waiver; obtain community resources.
• Serve as liaison between patients/families and community agencies.
• Collaborate with the clinical team to support holistic care in home settings.

Qualifications:

• Social Work Degree
  • Tennessee: Master's Degree in Social Work required.
• Current State Licensure in Social Work
  • Tennessee: LMSW or LCSW required.
• Minimum of one year's experience in health care or hospice setting

What You'll Do:

• Performs the psychosocial assessment of patients and families/caregivers in the hospice program in a timely manner and ensures that identified needs are addressed in the patient's plan of care. Provides an assessment in the patient's identified residence and assistance when this is not safe, and another plan is required
• Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers. Assist team members as needed to ensure that this level of service is consistently provided
• Counsels patient and family/ caregivers and staff as needed in relationship to stress, and other identified coping difficulties
• Provides accurate and timely documentation of patient services to reflect the plan of care
• Provides assistance with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver. Obtains resources from community social, health, pharmaceutical and welfare agencies to assist in meeting patient/family needs
• Serves as liaison between patients and families/caregivers and community agencies.
