Qualifications

• Bachelor's degree in social work or related field; Master's preferred
• LSW licensure required; LCSW/LICSW preferred
• Minimum of two years in a skilled nursing facility
• Experience with short-term rehabilitation and discharge planning
• Excellent communication, documentation, and assessment skills

Responsibilities

• Build positive relationships between residents and staff to support each resident's individual needs
• Identify options and resources that best meet physical, emotional, and psychosocial needs
• Maintain communication with residents' families regarding changes in health, goals, and care planning
• Make appropriate referrals and coordinate services with outside agencies and community resources
• Provide or arrange counseling and supportive services as needed
• Assist residents with financial and legal matters and connect them with resources
• Participate as a member of the interdisciplinary team to support comprehensive resident care
• Review facility policies to help ensure compliance with regulations

Job Summary

We are seeking a compassionate and dedicated Social Services professional to join our healthcare team. This position plays an important role in supporting residents and their families by addressing individual psychosocial needs, coordinating services, assisting with discharge planning, and promoting overall resident well-being. The ideal candidate will have experience working in a skilled nursing facility, strong assessment and communication skills, and a genuine interest in helping residents navigate their healthcare and personal needs.

Responsibilities
