Visiting Angels Jenkintown is hiring a Social Worker to join our Wellness Team. You will perform initial home visits and ongoing assessments for senior clients, and supervise caregivers to ensure safety and quality of care.

Qualifications

• Social Work degree from an accredited school; 5+ years of experience preferred or LPN experience- 7+ years.
• Dementia care experience preferred
• Sales experience preferred
• Strong communication skills
• Ability to prioritize workload and problem solve
• Must have a yearly TB test
• Ability to multi-task effectively in a highly demanding environment
• Strong organizational and communication skills; detail oriented and able to delegate

Responsibilities

• Conduct preliminary home visits and ongoing assessments of senior clients.
• Perform in-home supervisory visits every 30 and 60 days for caregivers.
• Update the plan of care and make revisions as needed.
• Conduct safety checks in homes to ensure client safety.
• Supervise, train, and orient caregivers per guidelines.
• Identify and address caregiver performance issues as needed.
• Co-manage caregiver training programs and QA evaluations.
• Perform related duties as deemed appropriate by management

Visiting Angels Jenkintown is looking for experienced, compassionate, and professional Social Worker to join our Homecare team. In this role, you will complete preliminary home visits and on-going assessments of our senior clients. You will also complete supervisory checks of caregivers, ensuring the safety, wellness, and satisfaction of clients and their families.

Responsibilities:

• Ensure company standards as a quality provider through the implementation of excellent quality care and exceptional customer service
• Conduct Home Visits to bring new clients or reinstate old clients
• Conduct in-home supervisory visits every 30 and 60 days
• Update the plan of care and make necessary revisions as needed
• Conduct safety checks in the home to maintain a safe environment for clients
• Supervise, teach and orient caregivers per agency guidelines and regulatory standards
• Participate in the supervision of Caregivers; recognize Caregivers for a job well done
• Identify Caregiver performance issues and address appropriately
• Co-manage Caregiver training program
• Participate in the quality assurance evaluations of the services provided to the clients
• Perform related duties and responsibilities as deemed appropriate by the Management team

Requirements:

• Social Work related degree from an accredited school; at least 5years of experience, preferred or LPN experience- 7+ years
• Dementia care experience preferred
• Sales experience preferred

This position will be for our Jenkintown location. Our Jenkintown office is located at 1250 Greenwood Ave, Jenkintown, PA, 19046.

We serve Greater Philadelphia area- from Delaware border to Horsham, PA. This role supports clients in Mainline. This position requires travel to these areas and additional areas in our territory.
