Piedmont-Healthcare seeks a Social Worker for Piedmont Newnan Hospital to support patient and family wellness and navigate complex psychosocial needs in inpatient and Emergency Department environments.

The role emphasizes developing transitional care plans and coordinating with hospital resources to improve care outcomes in Georgia. MSW is required; LMSW preferred and licensure in Georgia is encouraged.

Responsibilities

The Social Worker strives to promote patient and family wellness, improved care outcomes, access to appropriate hospital and community resources, and manages, supports and develops comprehensive transitional care plans for patients with complex and psychosocial needs in Inpatient and Emergency Department (ED) environments.

Qualifications

Education

• Master’s Degree from an accredited social work program Required

Work Experience

• No experience required Required
• 2 years of experience in an acute or post-acute setting Preferred

Licenses and Certifications

• LMSW - Licensed Master Social Worker in the State of Georgia Preferred
