Job summary

Qualifications

• CCM - Certified Case Manager Preferred

Responsibilities

• Promote patient and family wellness, improve care outcomes, and manage and develop comprehensive transitional care plans for patients with complex and psychosocial needs in Inpatient and ED environments.
