Job summary

Piedmont Rockdale Hospital seeks a Social Worker to support inpatient and ED patients, promoting patient and family wellness, improving care outcomes, and coordinating access to hospital and community resources.

You will collaborate with physicians, care teams, patients, and families to develop comprehensive transitional plans for individuals with complex psychosocial needs. A Master’s in Social Work and 1 year of acute/post-acute experience are required, with LMSW/ACM/CCM preferred.

Qualifications

• Master’s Degree in Social Work from an accredited program.
• Minimum 1 year of experience in an acute or post-acute setting.
• Preferred LMSW/ACM/CCM licensure; none required.

• This position strives to promote patient and family wellness, improved care outcomes, access to appropriate hospital and community resources, and manages, supports and develops comprehensive transitional care plans for patients with complex and psychosocial needs in Inpatient and Emergency Department (ED) environments. Works to promote quality of care and effective transitions planning through collaboration with all service team members, physicians, patients, external agencies and families.
